Dance CoLab Waiver & Release of Liability

Applies To: Dance CoLab classes, socials, workshops, private or group lessons, practices, events, performances, check-in areas, rented venues, online registration, and related activities.

This Waiver & Release of Liability is a legal agreement between the participant, or the participant’s parent/legal guardian if the participant is under 18, and The Dance CoLab LLC, doing business as Dance CoLab including its owners, organizers, instructors, volunteers, contractors, staff, venue partners, assistants, photographers, videographers, and representatives.

By registering for, attending, observing, volunteering at, or participating in a Dance CoLab activity, I agree to the following:

Dance CoLab events include social dancing, lessons, workshops, practice, movement, physical contact, music, crowded spaces, and venue activity. These activities involve normal risks, including slips, falls, collisions, muscle strain, illness exposure, emotional discomfort, or other injury.

By participating, you agree to take responsibility for your own safety, follow event rules, respect others, and release Dance CoLab and related parties from claims to the fullest extent allowed by law.

I understand that participation in Dance CoLab activities is voluntary. I may choose not to participate in any activity, exercise, dance, partner rotation, movement, or interaction.

I understand that I am responsible for deciding whether I am physically, mentally, and emotionally able to participate safely.

I understand that dance and event participation may involve risks, including but not limited to:

• Slips, trips, falls, collisions, or contact with other participants
• Muscle strain, sprains, soreness, fatigue, or physical injury
• Injuries caused by movement, lifts, dips, turns, styling, floor conditions, footwear, crowded spaces, or partner connection
• Illness exposure, including contagious disease
• Emotional discomfort, interpersonal conflict, or unwanted interaction
• Lost, stolen, or damaged personal property
• Risks related to venue conditions, parking, weather, travel, or third-party services

I knowingly and voluntarily accept these risks.

I agree to:

• Follow Dance CoLab rules, venue rules, and instructor directions
• Respect other participants’ boundaries, consent, and personal space
• Use reasonable care for my own safety and the safety of others
• Avoid reckless, unsafe, aggressive, or inappropriate behavior
• Stop participating if I feel unsafe, unwell, injured, dizzy, overwhelmed, or unable to continue
• Notify Dance CoLab staff if I observe unsafe conduct or have a concern

I understand that Dance CoLab may remove or deny participation to any person for safety, conduct, or policy reasons.

I confirm that I am physically able to participate, or I have consulted with a medical professional if needed.

I understand that Dance CoLab does not provide medical advice, medical screening, or medical supervision.

I am responsible for managing my own health conditions, medications, allergies, injuries, limitations, and emergency needs.

To the fullest extent allowed by law, I release and hold harmless The Dance CoLab LLC, doing business as Dance CoLab, its owners, organizers, instructors, assistants, volunteers, contractors, staff, venue partners, representatives, successors, and assigns from claims, demands, damages, losses, liabilities, costs, or expenses arising out of or related to my participation in Dance CoLab activities.

This includes claims related to injury, illness, property loss, participation, observation, event attendance, venue use, or interaction with other participants, except where such release is not allowed by law.

If I experience a medical emergency and I am unable to communicate, I authorize Dance CoLab staff or representatives to seek emergency medical assistance on my behalf.

I understand that Dance CoLab is not responsible for medical costs, transportation costs, treatment decisions, or outcomes.

I agree not to attend Dance CoLab activities if I am knowingly contagious, have symptoms of a contagious illness, or have been advised to avoid group activities.

I understand that Dance CoLab cannot guarantee that I will not be exposed to illness.

I am responsible for my own belongings. Dance CoLab is not responsible for lost, stolen, damaged, or unattended personal property.

Dance CoLab events may include photography or video recording for promotional, educational, archival, or social media purposes. 

I grant Dance CoLab permission to use photos or videos that include me for promotional, educational, social media, website, and archival purposes without compensation, unless I notify Dance CoLab in writing before the event and Dance CoLab confirms an accommodation.

If the participant is under 18, this agreement must be completed by a parent or legal guardian.

The parent or legal guardian confirms that they have authority to sign for the minor participant and agrees to supervise or be responsible for the minor’s participation as appropriate.

By typing my full legal name and submitting the waiver form, I agree that my electronic signature has the same intent as a handwritten signature.

I consent to completing this waiver electronically and understand that Dance CoLab may store an electronic record of my submission, including the submitted name, email address, date/time, IP address, waiver version, policy version, and related registration information.

If any part of this agreement is found unenforceable, the remaining parts will remain in effect to the fullest extent allowed by law.

This agreement is intended to be governed by the laws of the State of Indiana, unless another law is required to apply.
